history. This information may be obtained from an application or claim
form, or it may be acquired from other sources, such as medical forms or

to administer to your insurance needs and strictly as permitted by law.
Safeguarding
your Medical Information:
We
assure you that we do not rent, sell or give your medical information to
anyone. It is used strictly fur the purposes of underwriting for your
insurance needs and where permitted by law.

we may Disclose Information:
We
respect the confidentiality of your personal information. Your information
is only shared where permitted by law and used for the underwriting or
administering of your insurance needs. It may be shared with insurance
company underwriters for the purposes of writing your insurance, or with
auditors, consultants and claims representatives directly relating to your
insurance account and needs. The information is not sold or rated.
Information
on former clients is also not disclosed without your approval and is not
